Summer Program Forms

Submit forms via email to dkbaylis@udel.edu, fax to 302-831-3105, or mail to UD Lab School, 459 Wyoming Road, Newark DE 19716. You may also deliver them in person to our front desk.

You may type directly into the forms, except for the health appraisal, which your pediatrician will need to complete. If you prefer hard copies of these forms, please let us know, and we’ll get them to you.

Summer Program tuition is due in full by April 1 – we accept checks payable to the UD Lab School, or you may click here to pay online.
